If your company has an active or recently expired membership, you don't need to fill out an application.
You can easily pick up where you left off from your NAMM Dashboard here: https://dashboard.namm.org/
Once logged in, you need to ensure your account has Admin or Billing access. Then you should see a Renew now button.
After selecting Renew now, you'll then be prompted to purchase with a credit card or bank account to renew your membership.
